Bausch & Lomb (Aust) Pty Ltd, (‘Bausch + Lomb’), hereby informs you that it collects and uses personal information about you.
The Personal Data you provide (Name, Email Address and Mobile Phone Number) will be passed on to the optometry practice (“Stockist”) you have selected to advise that you may be visiting their premises soon to receive a trial of Bausch + Lomb Ultra® ONE DAY Contact Lenses. The Stockist may require you to have an eye test before the free trial pack of Bausch + Lomb Ultra® ONE DAY Contact Lenses is provided, or you may need to provide verification that you have had an eye test in the last 12 months and/or provide your prescription details. You may be required to pay the Stockist for this eye test personally and Bausch + Lomb will not re-imburse you for this.

By submitting your Personal Data, you are consenting to Bausch + Lomb to collect your Personal Data. Nonetheless, you may decline but if you choose not to provide your Personal Data it will not be possible for us to provide you with a free trial voucher for Bausch + Lomb Ultra® ONE DAY Contact Lenses. In accordance with applicable law, you have certain rights in respect of your personal information, such as a right of access, rectification, restriction, erasure, opposition, and portability. To exercise those rights, please contact Bausch & Lomb at: DPO@bausch.com.  If you are not satisfied with Bausch & Lomb’s response or have concerns about Bausch & Lomb’s data processing practices, you can contact Bausch & Lomb’s Data Protection Officer at: DPO@bausch.com.  You also have a right to lodge a complaint with the Supervisory Authority for data protection in your country. You should continue to use UV-absorbing eyewear as directed.
